Honouring our care champions
Champions of Community Care recognizes community care professionals who go above and beyond in service, compassion, and dedication to the people they support. Across Canada, home healthcare is the thread that holds our healthcare system together and that dedication deserves more than our appreciation. It deserves to be celebrated.
The Access Care Foundation is proud to present the inaugural Champions of Community Care Recognition & Awards Gala, with one purpose: to ensure our home healthcare heroes are seen, heard and honoured.
- Open to everyone. Patients, families, friends, colleagues, peers and employers can all nominate.
- Frontline & allied health. Nurses, support workers, therapists and frontline staff who deliver care at home.
- Champlain / Eastern Ontario. Celebrating those who care for our community, close to home.

Who can be nominated?
Frontline home and community care professionals — including nurses, personal and home support workers, allied health professionals and frontline operations staff — who primarily deliver care in a community-based homecare setting.
Who can submit a nomination?
Anyone who has witnessed excellence firsthand: patients, families, friends, colleagues, peers and employers are all welcome to nominate a champion.
